Employee Wellness Plan

 

Over the past two years, the Healthy Choices Committee has been working to develop a comprehensive wellness program for District employees.  This program is designed to allow all employees the choice to participate in order to receive a discount on his or her health insurance premium share. 

Members of the Administrative and Support group, as well as Secretaries/Clerks/Aides (AFSCME Local 938) will participate in a point system for the 2009-10 school year.

Members of the Teacher Group (JEA) are only required to complete a Health Risk Assessment (HRA) at this time.  All other requirements are pending contract negotiations.

Teachers:

The only requirement for 2009-10 is to complete an health risk assessment (HRA). Pending contract negotiations may change this requirement in the future.

Administrative & Support Staff:

Employees will need to earn his or her insurance premium discount by earning 100 points by participating in wellness activities.  Click here for Wellness Points Chart. (PDF)

  • The 100 point total will need to be earned from June 1, 2009 through April 30, 2010.
  • Employees will need to record their points at www.mylearningplan.comClick here for instructions on how to record your points on My Learning Plan.
  • Ten percent of the participating employees will be randomly selected for an audit at the end of April. These individuals will be contacted to submit documentation supporting the participation in their recorded wellness activities. The paperwork will need to be submitted by May 31, 2010. The documentation could include things such as insurance Explanation of Benefits indicating a service was received, and exercise log, a log of dates and times of participation in District or community wellness events, etc.
  • At this time we will not do any physical testing to verify that participants are not using tobacco products. 
  • If participants do not earn and record their 100 point requirement by April 30, their payroll deduction will be changed and back premiums collected over their remaining paychecks for the school year. No "reminders" will be sent. 
  • If you are not interested in participating in the wellness plan, please contact Tonya Williams to complete a waiver form. The waiver must be turned in by July 1, of the year it applies to.

Custodial/Maintenance & Foodservice:

There is no wellness requirement for the 2009-10 school year. Pending contract negotiations may change in the future.
